Comprehensive Care for the family.
Membership Includes:
Annual Comprehensive Physical Exam
Preventative Care including nutritional counseling, weight loss, and smoking cessation
Management of chronic conditions and medications (excluding controlled substances)
Acute Care for illness and injuries (excluding Emergency Care)
Virtual Visits by email, text and phone
In-office tests including EKG, Spirometry, Urinalysis, Rapid Strep tests
Amazing Lab Discounts
Minor procedures (excluding charges for pathology)
Membership
There here is a one time application fee for new enrollment of $125
Individual
Any single individual is $85 per month.
Veterans
We deeply appreciate the sacrifice made by the men and women who have served our country, and we’re pleased to offer a 10% discount on any membership as a token of our gratitude.
Couple
Whether husband and wife, mother and son, father and daughter, this pricing is for you. The price per couple is $160 per month.
Business Plan:
$80 monthly with an initial discounted $75 enrollment fee per employee (with enrollment of 10 or more employees).
Family
A family with 2 children between the ages of 13-18 years old is $200 per month and $35 for each additional child.
