  • Yes, you can join as a patient if you have Medicare. However, your monthly membership fee cannot be billed to Medicare for reimbursement. You will need to sign a waiver every two years stating that neither you nor your doctor will seek reimbursement from Medicare for the membership fee. Medicare and supplemental insurance will continue to cover external medical costs, such as prescriptions from outside pharmacies, lab tests, hospital charges, physical therapy, and imaging services. Your Medicare coverage remains intact for other medical needs while you are a patient at Living Well Family Medicine.

  • If you choose to cancel your membership, we aim to handle the situation with transparency and respect. Should we be unable to address any concerns you raise, you may cancel your membership with a 30-day notice. If you decide to return to our practice, please note that rejoining will incur additional fees.
